Exam structure
The NCLEX® is computer-adaptive, so it has no fixed question count. NCSBN publishes the share of items drawn from each Client Needs category instead.

Clinical judgment
A case study is a set of six items built on one unfolding client presentation. Each item measures one step of the NCSBN Clinical Judgment Measurement Model, in this order.
Identify relevant and important information from different sources, such as medical history and vital signs.
Organize and connect the recognized cues to the client's clinical presentation.
Evaluate and rank hypotheses by urgency, likelihood, risk, difficulty and time constraints.
Identify expected outcomes and define a set of interventions to reach them.
Implement the solutions that address the highest priority.
Compare observed outcomes against expected outcomes.
Every NCLEX-RN® candidate answers 18 case-study items, which is three complete sets, plus roughly 10% stand-alone clinical judgment items.
Scoring
NCSBN states plainly that no fixed percentage of candidates passes or fails. You are measured against the passing standard, not against other candidates.
A candidate may pass or fail regardless of how many items they answered. Stopping at 85 is not good news, and reaching 150 is not bad news.
Once you confirm an answer and move on, it is locked. Every item must be answered before the next one appears, so a blind guess is better than a stall.
Where an item has more than one correct key, NCSBN scores it for partial credit using plus/minus, zero/one or rationale scoring.
