The tools that move a score
Everything else on this page supports these. If they are not good, nothing else matters.
A score tells you that you were wrong. A rationale tells you why, and why the answer you picked looked right. That difference is where practice turns into a higher score, so it is the thing we invest the most in.

Why
Left-sided failure backs blood up into the pulmonary circulation, so fluid crosses into the alveoli and you hear crackles. Jugular venous distension is systemic congestion, which is the right side.
Learning a topic and rehearsing an exam are different jobs, and one mode cannot do both. So the same questions are reachable three ways, and you move between them as your date gets closer.

Why is furosemide held when the potassium is 3.1?

Furosemide is a loop diuretic, so it makes the client lose more potassium. At 3.1 they are already below range. Monitoring after the dose does not undo the loss you caused, which is why holding and reporting comes first.
Getting a question wrong tells you a concept is weak. One more attempt at the same item only tells you whether you remember the answer. So on review you can generate fresh questions on that exact concept and find out whether you actually understand it.
